All players must have a valid physical with all signatures in order to begin tryouts. Physicals are only good for one school year and must be dated after April 15 of the previous school year.

Pay-to-Participate Consent Form

If you would like to request a full or partial scholarship for the pay-to-participate fee, please fill out the following form and return it to the Athletic Office.

Scholarship Request Form

You can fill out both the Statement of Eligibility Form and the Pay-to-Participate Consent Form as well as pay the Pay-to-Participate Fee ($175) using a new online system. You can even pay with a credit card!
